  • Part-time, Full-time Job Overview Join our compassionate team as a Hospice Home Health Aide, where your dedication and caregiving skills will make a meaningful difference in the lives of patients receiving end-of-life and palliative care.
In this vital role, you will provide personalized, respectful, and comprehensive home health services to individuals facing complex health challenges. Your energy and empathy will help promote comfort, dignity, and independence for patients in their own homes. This position offers an opportunity to work closely with interdisciplinary teams, utilizing your caregiving expertise to deliver exceptional patient-centered care in a supportive environment. Responsibilities Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, mobility, and toileting to ensure comfort and safety. Monitor and document patient condition through patient observation, vital signs measurement, and medical documentation within Electronic Health Record (EHR) systems. Administer medications and perform catheter care following medical orders while maintaining compliance with HIPAA regulations. Support patients with dementia, Alzheimer's disease, memory care needs, and those requiring hospice or hospice care services. Utilize equipment such as Hoyer lifts and assist with transfers to prevent injury and promote mobility. Assist with meal preparation, feeding, light cleaning, laundry, and maintaining a safe home environment for the patient. Collaborate with nursing staff and other healthcare professionals to update medical records and ensure continuity of care. Provide compassionate support tailored to individuals with developmental disabilities or long-term care needs while respecting their dignity and preferences. Experience Prior experience working in assisted living facilities, nursing homes, or home health environments is highly preferred. Demonstrated proficiency in patient observation, vital signs monitoring, medication administration, and medical terminology. Experience working with individuals with disabilities or special needs such as dementia or Alzheimer's is advantageous. Familiarity with EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems for accurate medical documentation. Certification in CPR is required; additional certifications such as hospice care training or catheter care are a plus. Proven ability to handle home & community care tasks including meal prep, cleaning, laundry, and providing emotional support in a live-in employment setting is desirable.
