Responsibilities:

• Provide direct patient care in the home setting
• Assist with activities of daily living, such as bathing, dressing, and grooming
• Administer medication as prescribed by the healthcare team
• Monitor and record vital signs and report any changes to the nurse or healthcare provider
• Assist with mobility and transfers
• Perform light housekeeping duties, including cleaning and organizing the patient's living space
• Maintain accurate and up-to-date medical records
• Provide emotional support and companionship to patients

Experience:

• Previous experience in caregiving or home health aide role preferred
• Familiarity with medical terminology and basic healthcare procedures
• Knowledge of memory care techniques for patients with dementia or Alzheimer's disease
• Experience with tube feeding, catheter care, and other specialized care procedures
• Understanding of hospice care principles and practices

Skills:

• Excellent communication and interpersonal skills to interact effectively with patients, families, and healthcare team members
• Strong attention to detail to ensure accurate documentation and medication administration
• Ability to follow instructions and work independently while adhering to established care plans and protocols
• Proficient in using electronic medical record systems or other healthcare documentation systems
