Job Description
Position Summary The Hospice Certified Home Health Aide (CHHA) provides personal care and supportive services to hospice patients in accordance with the patient's individualized plan of care under the supervision of a Registered Nurse. The CHHA delivers compassionate, dignity-centered care to patients residing in assisted living facilities, long-term care facilities, and private homes throughout the community. This field-based position requires regular travel between patient locations. Essential Duties & Responsibilities Provide hands-on personal care including bathing, grooming, oral care, dressing, toileting, incontinent care, skin care, and ambulation assistance Assist patients with transfers, repositioning, and mobility support while following safety protocols Observe and report changes in patient condition, behavior, safety concerns, or environment to the supervising nurse promptly Provide emotional support and companionship to patients and families during end-of-life care Maintain accurate and timely documentation of patient visits and care provided Follow infection prevention and control procedures at all times Assist with light homemaking tasks related to patient care as directed by the plan of care Respect patient rights, confidentiality, dignity, and cultural preferences Communicate professionally with nurses, facility staff, families, and interdisciplinary team members Travel daily between patient homes, assisted living facilities, and long-term care settings within the assigned coverage area Maintain compliance with hospice regulations, company policies, and state CHHA requirements Qualifications Current Certified Home Health Aide (CHHA) certification in the State of New Jersey Previous hospice, home health, long-term care, or assisted living experience preferred Compassionate, dependable, and patient-focused demeanor Ability to work independently in the field with strong time management skills Excellent communication and interpersonal skills Ability to safely lift, reposition, and assist patients as needed Reliable transportation required Requirements Valid driver's license with an acceptable driving record Active automobile insurance Ability and willingness to travel between multiple patient locations daily Current CPR certification preferred Must successfully complete background check, health screening, and any required compliance training Work Environment This is a field-based position requiring travel to patient residences, assisted living communities, and long-term care facilities. The role may involve exposure to varying home environments, infectious diseases, and emotional end-of-life situations.
