Job Description

Job Summary We are seeking a compassionate and dedicated Certified Home Health Aide (CHHA) to provide exceptional in-home care to clients across various settings. As a vital member of our healthcare team, you will assist individuals with their daily living activities, monitor their health status, and ensure their comfort and safety. Your role is essential in promoting independence and enhancing quality of life for those receiving home health services. This position offers an opportunity to make a meaningful difference in the lives of patients while working in a supportive and dynamic environment. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor and record vital signs such as blood pressure, pulse, temperature, and respiration rates Administer medications accurately following physician instructions and document all medical records in Electronic Health Record (EHR) systems Provide specialized care for patients with dementia, Alzheimer's disease, or other memory-related conditions, including memory care techniques Support clients with medical procedures such as catheter care, wound management, and Hoyer lift transfers Assist with meal preparation and nutritional needs while maintaining proper food safety standards Perform light housekeeping tasks such as cleaning, laundry, and maintaining a safe environment to promote overall well-being Document patient care activities thoroughly in compliance with HIPAA regulations and organizational policies Collaborate with nursing staff and other healthcare professionals to ensure comprehensive patient care plans are followed Support hospice care patients by providing compassionate emotional support and comfort measures during end-of-life stages Qualifications Valid certification as a Home Health Aide recognized by the state Proven experience working with individuals with disabilities or in assisted living environments Knowledge of medical terminology, medication administration, and vital signs monitoring Experience with EMR (Electronic Medical Records) or EHR systems for accurate documentation Ability to assist with complex medical procedures such as catheter care and hospice support CPR certification preferred; additional certifications like dementia or Alzheimer's care are a plus- We will train Strong communication skills and the ability to work independently while following detailed care plans Prior experience in long-term care settings such as nursing homes or home health environments is highly desirable Join our team to deliver compassionate home health services that truly make a difference! Pay:
$18.00 - $20.00 per hour
