  • Social Worker
  • PRN The Social Worker is responsible for, but not limited to: 1.
Initiates action and obtains resolution. Responds promptly, professionally and patiently. Work Collaboratively. 2. Nurture by supporting all customers. Acts with courtesy, care, concern and compassion. Educates others by sharing knowledge and expertise. 3. Greets customers in a friendly and welcoming manner. Creates a positive first impression. Smiles, projects a positive attitude. 4. Through the continuous improvement of standards of practice and by providing a safe and secure environment the customer will be assured a sense of safety and well-being, free from risk and harm. 5. Contributes to the effectiveness of the department/Agency by remaining within the acceptable standard of the Agency's Attendance/Tardiness Control Policy. 6. Provides direct social work services in the home which meet Agency and professional standards. Meets Agency standards of competency. 7. Accurately documents care provided and maintains patient records according to Agency policies. 8. Assists with Agency and community programs/committees.
Education:
Master's Degree in
Social Work Professional Experience:
Current Social Work License in the State of New Jersey, LCSW preferred 1 year of post graduate experience in a health setting #APPLOIOTHER
