This vital role offers an opportunity to make a meaningful difference in the lives of individuals requiring personalized care and support. As a Home Health Aide, you will provide compassionate assistance with daily activities, monitor patient health, and ensure a safe, comfortable environment. Bilingual skills are preferred to better serve diverse communities, and all team members are valued for their commitment to quality care. This paid position is ideal for compassionate caregivers eager to grow their skills in a dynamic, supportive setting. Responsibilities Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and document patient vital signs and health status using Electronic Health Record (EHR) systems Support patients with medication administration and catheter care following medical guidelines Provide companionship and emotional support to individuals with dementia, Alzheimer's disease, or other cognitive impairments Assist with light housekeeping tasks such as cleaning, laundry, and meal preparation to maintain a safe and hygienic environment Use medical terminology accurately when documenting patient records and communicating with healthcare teams Operate medical equipment like Hoyer lifts safely and effectively during patient transfers Follow HIPAA regulations strictly to protect patient privacy and confidentiality Collaborate with nursing staff on hospice, memory care, or long-term care plans as needed
Requirements Bilingual :
English & Spanish Valid certification as a Home Health Aide or equivalent credential; certification must be current and active Prior experience in assisted living facilities, nursing homes, or home health environments preferred Knowledge of medical terminology, vital signs monitoring, medication administration, and infection control protocols Experience working with individuals with developmental disabilities or dementia is highly desirable Ability to operate EMR/EHR systems for accurate documentation of patient care activities Strong communication skills; bilingual abilities (English/Spanish) are preferred to serve diverse client populations Compassionate attitude with a commitment to providing respectful, patient-centered care Join us in delivering exceptional home health services that empower individuals to live safely and comfortably at home.
