SUMMARY OF JOB

Provides personal care and assistance to Home Care and Hospice clients under the supervision of the Case Manager and Clinical Manager.

RECRUITMENT REQUIREMENTS RECRUITMENT REQUIREMENTS

High School diploma or equivalent preferred; Must be able to read and understand written instructions; Must be able to take direction via telephone in patient's home; Must be able to make meaningful observations and report changes in patient condition; Must have basic knowledge of computer applications and ability to enter patient information into an electronic device. Successfully pass all competency exams.

LICENSE AND REGULATORY REQUIREMENTS

Qualified for employment under current state (Pennsylvania or New Jersey) state regulations, current driver's license and access to a car, current car registration, and current car insurance. • Eligible 1st of the month following start date. Retirement Savings with Employer match Up to $8000 Tuition Reimbursement PTO/ESL (Extended Sick Leave)
• Both can be carried over. Mileage Reimbursement https://careers.redeemerhealth.org/benefits Rates starting at $18.00/hour to $22.51/hour
• (hiring rate based upon years of experience and internal equity).
