Job Description

Overview The Home Hospice Aide is a paraprofessional member of the interdisciplinary group who works under the supervision of a registered nurse (RN) and performs various services for a patient as necessary to meet the patient's personal needs and to promote comfort. The Home Hospice Aide is responsible for observing the patient, reporting the observations and documenting the observations as well as the care performed.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES 1.
Perform personal care needs and attend to the patients' requests promptly. If unable to perform a certain task, report it to the Patient Care Manager and the RN Case Manager 2. Provide patients and families/caregivers with positive communication techniques, within your scope while maintaining ethics, respect, and confidentiality. 3. Meet the needs of patients by using equipment safely and properly. 4. Give personal care, including but not limited to, baths/showers, back rubs, oral hygiene, shampoos/conditioners, and changing the bed linen as indicated in the Aide care plan. 5. Assist in dressing/undressing the patients as needed. 6. Assist in feeding as needed. 7. Provide proper care and observation of patients' skin to prevent breakdown. 8. Report on the patients' condition and significant changes to the RN Case Manager. 9. Communicate to the families/caregivers regarding the patients' condition and interpersonal issues. 10. Assist in ambulation as needed. 11. Provide normal range of motion and transfers/positions as directed. 12. Assist with bed pans/urinals. 13. Assist with light laundry as needed. 14. Keep patients' living area clean and orderly as needed. 15. Adhere to charting requirements, care procedures, and standards of personal and professional conduct. 16. Respect patients' and families/caregivers' environment and personal needs. 17. Participate in Quality Assurance Performance Improvement (QAPI) activities. 18. Attend required company meetings. 19. Complete necessary training per licensure and the company. 20. Other duties as assigned.
KNOWLEDGE AND ABILITIES 1.
Must have knowledge of disease processes, including end of life concerns. 2. Must operate with cultural sensitivity with respect to working with people and groups of diverse, racial, ethnic, economic, and social backgrounds. 3. General computer skills required. 4. Experience with EMR systems preferred.
SKILLS 1.
Excellent oral, written, listening and communication skills. 2. Ability to work autonomously and collaboratively with all interdisciplinary teams. 3. Can multitask and work under pressure. 4. Excellent organizational skills. 5. Excellent time management skills. 6. Professional appearance and excellent customer service skills. 7. Understand hospice philosophy. 8. Comfortable providing end of life care.
POSITION QUALIFICATIONS 1.
High school diploma or equivalent. 2. Board certified nursing assistant. 3. CPR Certification 4. Fingerprint Card 5. Must be a licensed driver with an automobile that is insured and in accordance with the state/and or organization requirements and is in good working order.
WORK ENVIRONMENT 1.
Prolonged or considerable walking or standing. 2. Able to lift, position, and/or transfer patients. 3. Able to lift supplies and operate equipment, as necessary. 4. Considerable reaching, stooping, bending, kneeling, and/or crouching. 5. Visual acuity and hearing to perform required nursing skills. 6. Skilled Nursing Facilities (SNFs)/Group Homes and personal residence. 7. Considerable driving and in/out of vehicle. 8. Possible exposure to infectious diseases.
