This energetic position offers the opportunity to make a meaningful difference by providing clinical support, counseling, and case management to diverse populations, including seniors, individuals with disabilities, and those undergoing recovery or coping with mental health challenges. You will collaborate with interdisciplinary teams to develop personalized care plans, conduct assessments, and ensure comprehensive support for patients navigating complex health and social needs. This paid position is designed for motivated professionals eager to bring positive change into the lives of others through dedicated home health services. Duties Conduct thorough patient assessments to evaluate physical, emotional, and social needs, including diagnostic evaluations and mental health screenings Develop and implement individualized care plans that incorporate behavioral health strategies, counseling techniques, and community resources Provide clinical counseling and psychotherapy services such as behavioral therapy, motivational interviewing, and crisis intervention tailored to each patient's circumstances Manage case documentation accurately within electronic medical record (EMR) systems while adhering to HIPAA regulations for patient privacy and confidentiality Coordinate with healthcare providers, families, and community agencies to facilitate seamless home & community care delivery Offer specialized support for populations such as seniors requiring dementia or Alzheimer's care, children with developmental disabilities or autism, and individuals recovering from post-acute or hospice care Assist in intake processes, medical record management, CPT coding (Current Procedural Terminology), and ICD coding (International Classification of Diseases) for billing and documentation purposes Skills Proven experience in child & family counseling, hospice care, or behavioral health settings with a focus on mental health counseling or clinical research Strong knowledge of medical terminology, medical documentation standards, and HIPAA compliance for safeguarding patient information Skilled in conducting diagnostic evaluations, group therapy sessions, individual counseling, and crisis management techniques Familiarity with EMR systems and inpatient/outpatient clinic workflows; experience working with patients in assisted living or nursing home environments preferred Ability to perform comprehensive patient assessments utilizing tools like
ICD-10/ICD-9
codes; experience with CPT coding advantageous Excellent communication skills for engaging patients during intake and providing culturally sensitive clinical counseling across diverse populations Demonstrated ability to work effectively with individuals experiencing PTSD or dementia; experience working with children or individuals with disabilities is highly valued Embark on a rewarding career path where your expertise directly enhances the well-being of those in need.
