Overview We are seeking energetic and dedicated Home Health Aides to join our team. In this vital role, you will deliver personalized care to clients in their homes, ensuring their safety, comfort, and well-being. This position offers an opportunity to make a meaningful difference in the lives of those you serve while working in a supportive environment that values your skills and compassion. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and observe patients' conditions, recording vital signs and reporting changes promptly Support clients with medication administration and manage medical documentation accurately using Electronic Health Record (EHR) systems Provide specialized care for individuals with dementia, Alzheimer's, or other memory-related conditions, including memory care techniques Utilize medical terminology appropriately when documenting patient information and communicating with healthcare teams Operate assistive devices such as Hoyer lifts and perform catheter care when necessary Assist with household tasks such as cleaning, laundry, meal preparation, and light housekeeping to ensure a safe living environment Requirements Valid certification as a Home Health Aide (HHA) or equivalent in NY Proven experience in home health, assisted living, nursing homes, or long-term care settings Strong patient observation skills and ability to work with people with disabilities or developmental disabilities Knowledge of HIPAA regulations to ensure patient confidentiality and privacy Certification in CPR and first aid required; additional certifications such as hospice care or dementia training are a plus Familiarity with EMR/EHR systems for accurate medical documentation and record keeping Ability to work independently while following care plans meticulously Compassionate attitude with excellent communication skills to collaborate effectively with clients and healthcare professionals Join us in delivering compassionate, professional care that truly makes a difference!
