The Dieteic and Diabetes Prgram Care Assistant (DDPC) assists the Dietetic-Diabetes Care Team at CNY Family Care by performing patient support tasks and serving as a primary administrative resource for the Registered Dietitian (RD) and/or CDCES (Certified Diabetes Care and Education Specialist). Key responsibilities include coordinating patient scheduling, triaging patient and provider inquiries, processing diabetes-related supplies and durable medical equipment orders and supporting access to diabetes care services. Works cooperatively and communicates effectively within a multidisciplinary framework to actively promote holistic care for patients and families. Dietetic and Diabetes Program Care Assistant responsibilities Appointment management of the Dietetic-Diabetes Care Team. Directs patients for procedures/testing as ordered by the physician and/or the
RD/CDCES.
Gives patient and family information/education materials as directed by the RD/CDCES/Physician for the success of overall patient care. Assists patients in navigating access to care services such as insurance coverage, prior authorization for diabetes prescription and services, Patient Assistance programs, community resources. Coordinates compliance of Diabetic Retinopathy quality measures for entire value-based payor population and provides reporting on data relevant to the assigned patient population. Manages the diabetic retinal eye exam schedule daily and administer patient exams. Runs reports to identify patients due for an annual retinal eye exam and adds alert to appointment comment line. Reviews results for diabetic retinal eye exams performed daily and adds appropriate billing codes to E-superbill. Dietetic and Diabetes Program Care Assistant education/experience High school diploma or general education degree (GED) required; one to two years of college preferred. Minimum of two years previous experience working in Health Care required; or three or more years of related experience and/or training in a medical office which uses an Electronic Medical Record; or equivalent combination of education and experience. Prior experience using Retinavue technology preferred.
