Job Description

Overview Join our dedicated team as a Home Health Aide in Rockland County, Orange County, Dutchess County and Westchester County and make a meaningful difference in the lives of those you serve! This vital role offers the opportunity to provide compassionate, personalized care to individuals in their homes or assisted living settings. As a Home Health Aide, you will be the trusted support system for patients, helping them maintain independence, comfort, and dignity. Your energy and dedication will help foster a positive environment where clients feel valued and cared for every day. This position is paid and offers rewarding experiences working with diverse populations, including seniors, individuals with disabilities, and those requiring specialized dementia care. Duties Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary Monitor patient health by observing vital signs and reporting changes promptly to healthcare professionals Support patients with memory care needs, including Alzheimer's and dementia care, ensuring a safe and calming environment Provide companionship and emotional support to enhance overall well-being and reduce feelings of isolation Document medical records meticulously while adhering to HIPAA regulations to ensure patient confidentiality at all times Skills Strong patient observation skills with the ability to recognize changes in condition or behavior Ability to work with individuals with developmental disabilities or other special needs with patience and compassion Excellent communication skills to collaborate effectively with healthcare teams, patients, and families.
