Wantagh, NY Available Shifts :
40 hrs/wk: 11:00p•9:00a•Wednesday-Saturday 20 hrs/wk: 11:00p-9:00a•
Friday & Saturday Salary Range :
$22.57/hr
DSP Level :
DSP-4•Approved for
Driving and AMAP Hourly Rate :
$22.57/hr

Job Responsibilities:
Drive consumers to and from program in AM and/or PM, to medical appointments, recreation outings, or other destinations as needed or requested. Assist in recreation activities. Work directly with residents to implement individual treatment plans, log progress and activities. Work with therapeutic and nursing staff to coordinate resident services. Provide direct care services for residents, such as toileting, feeding, dressing, sorting of laundry, etc. Attend in in-service training to ensure implementation of the formal and informal goal plan. Clean residents' rooms, vacuum dust, change bed linens, make beds, empty trash buckets, clean bathrooms, do laundry, etc. Observe residents' bodies on a daily basis, if required, and report all physical conditions including hair, nails, sores, insect bites, rashes, etc. Administer medications in accordance with AMAP guidelines. Contact families as needed. Report situations/issues involving consumers to proper authorities. Incorporated Code of Ethics, Core Competencies and Empowerment Strategies in their daily professional responsibilities.
Physical Demands:
