This role offers an energizing opportunity to provide personalized, respectful, and high-quality care in a variety of settings, including private homes, assisted living facilities, and nursing homes. Whether you're seeking a flexible schedule or a full-time position, you'll be empowered to deliver essential services that promote independence and well-being. All positions are paid, recognizing the valuable contributions you bring every day. Duties Assist clients with activities of daily living (ADLs) such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary Monitor patient observations, including vital signs and overall health status, and document findings accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems Support medication administration following medical terminology guidelines and HIPAA regulations to ensure patient safety and confidentiality Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments through tailored memory care techniques Assist with catheter care, wound management, hospice support, and other complex medical procedures as trained Prepare nutritious meals and assist with meal planning while maintaining cleanliness and hygiene standards in the home or facility environment Perform light housekeeping tasks such as laundry, cleaning, and maintaining a safe environment to promote overall health and comfort Qualifications Prior experience in senior care, home health, assisted living, nursing homes, or working with individuals with developmental disabilities or disabilities preferred Knowledge of medical terminology, medication administration procedures, and vital signs monitoring required Certification in CPR (Cardiopulmonary Resuscitation) is highly desirable Experience with medical documentation systems such as EMR/EHR systems is a plus Ability to work independently while following detailed care plans and protocols Strong communication skills to effectively collaborate with clients, families, and healthcare teams Compassionate attitude with a genuine desire to improve clients' quality of life Valid driver's license and reliable transportation are preferred for home visits Join us in delivering exceptional patient care through dedicated service and compassionate support.
