Job Description

We are seeking a compassionate and dedicated Home Health Aide to provide exceptional in-home care to clients with diverse needs. The ideal candidate will possess a strong commitment to patient-centered care, with experience in assisting with activities of daily living (ADLs), medical documentation, and working within home health or long-term care environments. As a vital member of our caregiving team, you will deliver personalized support that promotes independence, safety, and well-being for individuals across various stages of health and mobility. Duties Assist clients with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary. Monitor and observe patients' conditions, accurately documenting vital signs, medication administration, and changes in health status using electronic health record (EHR) or electronic medical record (EMR) systems. Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments through tailored memory care techniques. Support clients in assisted living settings or their private homes by preparing nutritious meals, maintaining cleanliness, and managing laundry tasks. Administer medications following prescribed protocols while ensuring compliance with HIPAA regulations to protect patient privacy. Assist with medical procedures such as catheter care, hospice support, and wound management under the supervision of nursing staff. Engage with patients respectfully and empathetically, fostering a positive environment that encourages dignity and independence. Collaborate effectively with healthcare professionals to ensure comprehensive patient care and accurate medical documentation. Qualifications Must be a certified Home Health Aide (HHA) or Personal Care Aide (PCA). Must speak English fluently. Valid ID and Social Security Card. Two professional references.
