This position offers an exciting opportunity to make a meaningful difference by assisting clients with daily activities, ensuring their safety, and promoting independence. As a Home Health Aide, you will work closely with patients, families, and healthcare professionals to provide high-quality support that enhances quality of life. Your energy, empathy, and attention to detail will be essential in creating a positive and empowering environment for those in your care. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using equipment such as Hoyer lifts. Monitor patient health by observing vital signs and reporting changes to healthcare providers promptly. Administer medications and manage medical documentation in compliance with HIPAA regulations and EMR/EHR systems. Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments through tailored memory care techniques. Support patients with medical procedures such as catheter care, wound care, and hospice-related needs while ensuring comfort and dignity. Prepare nutritious meals and assist with feeding while adhering to dietary restrictions and medical instructions. Maintain a clean and safe environment by assisting with light housekeeping tasks like laundry and general cleaning. Document patient progress accurately in medical records and communicate effectively with interdisciplinary teams to ensure continuity of care. Use proper techniques for patient transfers and mobility assistance utilizing equipment like Hoyer lifts or other assistive devices. Engage with clients compassionately, providing emotional support and companionship to promote mental well-being. Experience Prior experience working in assisted living facilities, nursing homes, or home health settings is highly preferred. Demonstrated experience caring for individuals with developmental disabilities or dementia is advantageous. Familiarity with EMR/EHR systems for medical documentation is required; experience with home & community care is a plus. Proven ability to perform patient observation accurately and respond swiftly to changing needs or emergencies such as CPR certification or hospice care situations. Long-term care experience involving medication administration, vital signs monitoring, and use of medical equipment is desirable. Working knowledge of medical terminology relevant to home health caregiving roles is essential. Experience working with individuals who have disabilities or require specialized memory or Alzheimer's care enhances your candidacy.
