In this vital role, you will provide personalized assistance to individuals with diverse needs, promoting independence, comfort, and well-being in their own homes. Whether supporting seniors, individuals with disabilities, or those recovering from illness, your efforts will foster a safe and nurturing environment that encourages dignity and quality of life. Responsibilities Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts or transfer belts. Support patients with specialized care needs such as Alzheimer's disease, dementia, memory care, hospice care, catheter care, and working with individuals with developmental disabilities or disabilities. Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe living environment that meets health standards. Document patient progress and communicate effectively with healthcare teams regarding changes in condition or care plans. Utilize medical terminology confidently when recording observations or communicating with medical professionals to ensure clarity and accuracy. Skills Proven experience in home health care or long-term care environments with knowledge of medical procedures and documentation practices. Strong patient observation skills to detect changes in condition promptly and accurately. Experience working with individuals with disabilities or in assisted living settings is highly valued. Knowledge of medical terminology along with CPR certification to respond effectively in emergencies.
