Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary Monitor and observe patients' health status by taking vital signs such as blood pressure, pulse, and temperature, and report changes promptly Administer medications accurately following medical documentation protocols and HIPAA regulations to ensure patient safety Support patients with specialized needs such as dementia, Alzheimer's, memory care, or hospice care with patience and compassion Document all care activities thoroughly using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to maintain accurate medical records Prepare nutritious meals and assist with feeding while adhering to dietary restrictions and preferences Perform light housekeeping duties including cleaning, laundry, and maintaining a safe environment to promote overall well-being Use medical terminology correctly during documentation and communication with healthcare teams to ensure clarity and compliance Requirements Proven experience in home health care, assisted living, nursing homes, or long-term care environments Knowledge of patient observation techniques and ability to recognize changes in health status Familiarity with EMR/EHR systems for accurate medical documentation Certification in CPR and first aid is required; additional certifications such as medication administration or catheter care are preferred Experience working with individuals with developmental disabilities or dementia is highly valued Ability to work independently with strong organizational skills and attention to detail Compassionate attitude with excellent communication skills to connect effectively with clients and their families Must adhere to HIPAA guidelines ensuring confidentiality of patient information Join us in delivering exceptional home health care that transforms lives through dedicated service.
