Job Description

Job Overview As a Home Health Aide (HHA), you will play a vital role in enhancing the quality of life for individuals in need of assistance. Your compassionate care will empower clients to maintain their independence while receiving the support they require in their homes. You'll be responsible for providing essential personal care, assisting with daily living activities, and ensuring a safe and comfortable environment for those you serve. What you'll do Provide compassionate support to clients by assisting with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and meal preparation. Monitor and document vital signs, ensuring any changes in health status are communicated promptly. Assist clients with mobility using equipment like Hoyer lifts and provide catheter care as needed. Offer companionship and engage clients in meaningful activities to promote mental well-being, particularly for those with Alzheimer's or dementia. Maintain a clean and safe living environment by performing light housekeeping tasks including cleaning, laundry, and meal preparation. Administer medications as prescribed while adhering to HIPAA regulations and maintaining accurate medical documentation. Collaborate with healthcare professionals to develop personalized care plans that meet the unique needs of each client. Provide end-of-life support through hospice care when necessary, ensuring dignity and comfort for clients and their families. Basic qualifications Experience in patient observation and caregiving, particularly in home health or long-term care settings. Familiarity with Electronic Medical Records (EMR) systems and medical terminology. Certification in CPR and knowledge of vital sign monitoring techniques. Preferred qualifications Previous experience working with individuals with developmental disabilities or seniors requiring memory care. Proficiency in medication administration and understanding of hospice care principles. Strong communication skills, both verbal and written, to effectively document patient care and collaborate with team members.
