Job Overview Join our dedicated team as a Certified Home Health Aide, where your compassionate care and attention to detail will make a meaningful difference in the lives of individuals requiring assistance with daily activities. In this vital role, you will provide personalized support to clients in their homes, ensuring their safety, comfort, and well-being. Your expertise will help promote independence and improve quality of life for those with diverse health needs, including seniors, individuals with disabilities, and those receiving hospice or memory care. This position offers an engaging environment where your caregiving skills are valued and essential. Responsibilities Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts when necessary Monitor and document patient observations, including vital signs and changes in health status, ensuring accurate medical records in electronic health record (EHR) or electronic medical record (EMR) systems Support clients with specialized care needs such as Alzheimer's disease, dementia, hospice care, catheter care and memory care routines Perform light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe home environment to promote overall well-being Utilize medical terminology accurately when documenting patient information and communicating with healthcare teams Operate medical equipment such as Hoyer lifts and assist with medical procedures like blood pressure monitoring or medication administration Requirements Prefer Home Health Aide (HHA) or Certified Nursing Assistant (CNA), with current CPR certification Proven experience in home health care, assisted living facilities, nursing homes or long-term care environments Familiarity with EMR/EHR systems for documentation and patient record management Experience working with individuals with developmental disabilities, Alzheimer's or dementia care needs, hospice patients or those requiring memory care Knowledge of medical terminology and procedures related to patient care and safety standards Ability to work independently in a home setting while maintaining professional boundaries and compassionate communication Strong organizational skills for managing multiple client needs and detailed medical documentation Join us to be part of a passionate team committed to delivering exceptional home health services.
