This position offers an exciting opportunity to make a meaningful difference in patients' lives by assisting with daily activities, monitoring health conditions, and supporting overall well-being. As a Home Health Aide, you will work closely with clients, families, and healthcare professionals to deliver high-quality care that promotes independence and comfort. Duties Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts and other assistive devices. Monitor and record vital signs including blood pressure, pulse, temperature, and respiration to track health status accurately. Provide medication administration following prescribed instructions while maintaining strict adherence to HIPAA regulations for patient privacy and confidentiality. Support patients with specialized needs such as dementia care, Alzheimer's care, hospice care, and working with individuals with developmental disabilities or disabilities. Document medical records and care activities using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems efficiently and accurately. Assist with household tasks including meal preparation, cleaning, laundry, and maintaining a safe environment for clients. Support emotional well-being through compassionate patient observation and engaging in meaningful activities tailored to each individual's needs. Qualifications Proven experience in home health care, senior care, or long-term care environments; experience in nursing homes or assisted living facilities is highly valued. Knowledge of medical terminology, medication administration protocols, catheter care, and hospice or memory care practices. Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as dementia or Alzheimer's training are a plus. Ability to work independently while following detailed care plans and medical documentation procedures. Strong communication skills for working effectively with patients, families, and healthcare teams. Valid driver's license and reliable transportation preferred for live-in or community-based assignments.
