Responsibilities Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support Monitor patient health by observing vital signs and reporting changes to healthcare teams Support medication administration following prescribed protocols while maintaining HIPAA compliance and accurate medical documentation Provide companionship and emotional support to enhance patient morale and mental well-being Perform light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe living environment Utilize Electronic Health Record (EHR) or EMR systems to document patient care activities accurately Operate medical equipment such as Hoyer lifts, catheters, and other assistive devices safely and effectively Assist with specialized care needs including Alzheimer's care, dementia care, hospice support, and working with individuals with developmental disabilities or disabilities Requirements Proven experience in home health care, senior care, nursing homes, or assisted living environments Knowledge of medical terminology and procedures related to patient care Valid CPR certification and training in emergency response procedures Ability to perform patient observation accurately and respond promptly to changing conditions Experience working with individuals with disabilities or complex medical needs such as dementia or memory care patients Familiarity with EMR/EHR systems for documentation purposes Ability to lift patients safely using Hoyer lifts or other assistive devices when necessary Strong communication skills to collaborate effectively with healthcare teams and family members Valid driver's license preferred for live-in employment or community-based visits This role is ideal for caring professionals passionate about providing high-quality home health services.
