This vital role offers an opportunity to provide personalized, respectful care that promotes independence, comfort, and well-being. Whether working with seniors, individuals with disabilities, or those requiring specialized memory or hospice care, you will be at the forefront of delivering exceptional patient-centered services in a supportive environment. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using tools like Hoyer lifts and catheter care when needed Monitor and record vital signs such as blood pressure, pulse, temperature, and respiration to ensure patient health and safety Administer medications accurately following medical documentation protocols while maintaining compliance with HIPAA regulations Provide specialized care for clients with dementia, Alzheimer's disease, or other memory impairments through tailored memory care techniques Support clients with medical procedures such as wound care, hospice support, and working with EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems for documentation Prepare nutritious meals and assist with meal planning and cooking to promote healthy living in accordance with dietary needs Maintain a clean and safe environment by assisting with light housekeeping tasks including laundry and general cleaning Skills Strong patient observation skills to detect changes in condition or behavior promptly Experience working in assisted living facilities, nursing homes, or home health environments with long-term care experience Knowledge of medical terminology relevant to caregiving tasks and patient records management Proficiency in using EMR/EHR systems for accurate medical documentation and record keeping Ability to work effectively with individuals who have developmental disabilities or require dementia and Alzheimer's care Certification in CPR (Cardiopulmonary Resuscitation) and training in hospice care practices preferred Familiarity with medical equipment such as Hoyer lifts, vital signs monitors, catheters, and medication administration protocols Compassionate communication skills to build trust and rapport with clients and their families Flexibility to work live-in shifts or part-time schedules as needed in home & community care settings Join us to deliver heartfelt caregiving that empowers individuals to live their best lives.
