This is a role for someone dependable, patient, and service-driven who wants meaningful work and flexible opportunities. You'll be a trusted presence in the home - helping clients stay safe, comfortable, and independent. Your work directly supports quality of life for patients and peace of mind for their families. Key responsibilities Provide personal care support including bathing, grooming, dressing, oral hygiene, and toileting Assist with mobility, transfers, ambulation, and range-of-motion exercises per care plan Prepare meals and assist with feeding as needed Support medication self-administration through reminders (no independent med administration) Monitor and report patient condition and changes to the supervising nurse Take and record basic vital signs when trained and assigned Maintain a clean, safe, and orderly client environment Assist with linens and light client laundry as appropriate Use safety equipment properly (rails, supports, stools, etc.) Document care provided according to company and regulatory standards Travel reliably to patient homes and assigned visits across your service area Complete required in-service training, including annual dementia education What success looks like Patients feel safe, respected, and well cared for On-time arrival and dependable attendance for assigned visits Accurate, timely care documentation Positive feedback from patients and families Completion of required certifications and training on schedule Strong communication with supervising nurse and care team Minimum Qualifications High school diploma or equivalent. Completion of a state-approved Home Health Aide (HHA) training program, CNA certificate, STNA license or at least 1.5 years of recent caregiving experience in home care, assisted living, long-term care, nursing home, or hospital settings. HHA certification or equivalent credential as required by Ohio state regulations. Basic knowledge of patient care and safety protocols. Ability to pass required background checks and health screenings. Preferred Qualifications CPR and First Aid certification. Familiarity with Electronic Health Record (EHR) systems. Strong interpersonal and communication skills. Experience caring for seniors or individuals with chronic conditions, disabilities, or dementia.
