In this vital role, you will provide essential support to clients by assisting with daily activities, ensuring their safety, and promoting independence. Your dedication will help improve quality of life for those with various health conditions, disabilities, or age-related needs. This position offers an opportunity to work in a dynamic environment where your caregiving skills are truly valued and appreciated. Responsibilities Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support Monitor and observe patients' health status, including vital signs and overall condition, reporting any changes promptly Administer medications accurately following prescribed instructions while maintaining detailed medical documentation Support clients with specialized care needs including Alzheimer's care, dementia care, hospice support, catheter care, and memory care Operate Electronic Health Record (EHR) and Electronic Medical Record (EMR) systems to document patient information securely and efficiently Use medical terminology appropriately when recording observations and communicating with healthcare teams Assist with light housekeeping tasks such as cleaning, laundry, meal preparation, and maintaining a safe environment in compliance with HIPAA regulations Utilize equipment like Hoyer lifts safely to assist with patient transfers and mobility Provide compassionate care for individuals with developmental disabilities or long-term care needs in home or assisted living settings Support patients' emotional well-being through attentive listening and respectful interaction Qualifications Proven experience working as a Home Health Aide or in similar caregiving roles within home health, nursing homes, assisted living facilities, or long-term care environments Strong knowledge of patient observation techniques and medical terminology Certification in CPR (Cardiopulmonary Resuscitation) required; additional certifications such as hospice care or dementia training preferred Experience with medication administration, catheter care, hospice support, and using assistive devices like Hoyer lifts is highly desirable Familiarity with EMR/EHR systems for accurate medical documentation and record keeping Ability to work independently while following detailed care plans and protocols Compassionate attitude combined with excellent communication skills to work effectively with patients and their families Join us to provide exceptional home health care that truly makes a difference!
