This position offers an exciting opportunity to support clients with diverse needs, including senior care, disability assistance, and memory care. Your energetic and attentive approach will help promote independence, comfort, and well-being for those you serve. As a Home Health Aide, you will be an essential part of a team committed to delivering high-quality home and community-based care that makes a meaningful difference in clients' lives. Duties Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts or transfer belts. Monitor patient health by observing vital signs, reporting changes, and maintaining accurate medical documentation within Electronic Health Record (EHR) systems while adhering to HIPAA privacy regulations. Support medication administration under supervision, ensuring proper timing and documentation of all treatments. Provide specialized care for individuals with dementia or Alzheimer's disease through memory care techniques and tailored activities. Assist with medical procedures such as catheter care, wound dressing changes, and hospice support when needed. Perform light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe living environment for clients. Document patient progress diligently in medical records and communicate effectively with healthcare teams to coordinate ongoing care plans. Qualifications Proven experience working in assisted living facilities, nursing homes, or home health environments. Strong knowledge of medical terminology and patient care best practices. Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as medication administration or hospice care are a plus. Demonstrated ability to observe patients carefully and report findings accurately. Experience working with individuals with developmental disabilities or dementia is highly desirable. Familiarity with EMR (Electronic Medical Records) or EHR systems for documentation purposes. Ability to work independently while maintaining excellent communication with team members and families. Valid driver's license and reliable transportation preferred for live-in or travel assignments.
