Job Description

Job Overview We are seeking a compassionate and dedicated Home Health Aide to provide exceptional care and support to individuals in their homes. This vital role involves assisting clients with daily activities, ensuring their safety, and promoting independence. As a Home Health Aide, you will play a crucial part in enhancing the quality of life for those receiving home health services, working closely with families and healthcare teams to deliver personalized care that meets each client's unique needs. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts or walkers. Monitor and observe patients' conditions, recording vital signs and noting any changes or concerns to ensure prompt medical attention if needed. Support medication administration following proper protocols, including managing medical documentation and adhering to HIPAA regulations for patient privacy. Provide specialized care for clients with dementia, Alzheimer's disease, or other memory-related conditions, including memory care strategies and behavioral management. Perform light housekeeping tasks such as cleaning, laundry, meal preparation, and maintaining a safe environment to promote overall well-being. Document patient progress accurately using Electronic Health Record (EHR) systems or Electronic Medical Records (EMR systems), ensuring compliance with healthcare standards. Assist with medical procedures like catheter care, hospice support, or wound management under supervision or as trained. Support clients with disabilities or those in assisted living or long-term care settings by offering compassionate companionship and personalized care plans. Qualifications Prior experience in home health care, nursing homes, or assisted living facilities is highly preferred. Strong knowledge of medical terminology and patient care procedures. Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as medication administration or hospice care are advantageous. Experience working with people with developmental disabilities, dementia, Alzheimer's disease, or other cognitive impairments. Ability to work independently while following detailed care plans and instructions from healthcare professionals. Excellent communication skills for collaborating with clients' families and healthcare teams while maintaining confidentiality per HIPAA guidelines. Physical ability to lift patients safely using equipment like Hoyer lifts and perform routine caregiving tasks efficiently.
