Home Health Aide (HHA) & Direct Support Professionals (DSP)

Job Summary:

We are seeking compassionate and dedicated Home Health Aides (working with the elderly population) & Direct Support Professionals (working with individuals who have developmental disabilities) to provide essential care and support to individuals in their homes. As a Home Health Aide/Direct Support Professional, you will assist with daily activities, personal care, and ensure the well-being of our clients. Responsibilities:

• Provide direct patient care under the supervision of a registered nurse/manager
• Assist with personal hygiene tasks such as bathing, grooming, and dressing
• Monitor vital signs and report any changes or concerns to the nurse
• Assist with mobility and transfers, including using lifts or other assistive devices
• Support individuals in their daily activities
• Maintain a clean and safe environment for clients
• Document patient care activities accurately and timely in medical records
• Medication management as needed
• Assisting the individual to & from appointments & grocery stores

Requirements:

• Valid certification as a Home Health Aide or completion of an accredited training program or prior experience in the healthcare field.
• Experience working with individuals with DD/IDD and/or elderly population is preferred
• Ability to lift and move patients safely using proper body mechanics
• Compassionate and patient-focused approach to caregiving
• Excellent communication skills, both verbal and written
