Job Description
Job Overview Join our dedicated healthcare team as a Home Health Aide, where you'll play a vital role in providing compassionate, personalized care to individuals in their homes. This position offers an exciting opportunity to make a meaningful difference in the lives of patients by assisting with daily activities, monitoring health conditions, and supporting overall well-being. As a Home Health Aide, you'll work closely with clients and healthcare professionals to deliver high-quality care that promotes independence and comfort. This paid position is ideal for compassionate individuals eager to bring energy and dedication to home health services. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and observe patient health status, recording vital signs such as blood pressure, pulse, and temperature accurately in electronic health record (EHR) systems Support medication administration following prescribed protocols while maintaining strict adherence to HIPAA regulations for patient privacy Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory-related conditions, including memory care techniques and behavioral support Operate medical equipment such as Hoyer lifts, catheters, and other assistive devices safely and effectively Assist with meal preparation and nutritional needs, ensuring dietary restrictions are followed precisely Perform light cleaning, laundry, and home organization tasks to maintain a safe and comfortable environment for clients Document all patient interactions and medical records thoroughly using electronic medical documentation systems Support hospice care needs by providing compassionate companionship and symptom management as directed by healthcare providers Skills Strong patient observation skills to identify changes in health status promptly Experience working in assisted living facilities, nursing homes, or home health environments Proficiency with EMR (Electronic Medical Records) or EHR (Electronic Health Records) systems for accurate documentation Knowledge of medical terminology relevant to home health care settings Ability to assist with ADLs independently and compassionately while respecting patient dignity Certification in CPR (Cardiopulmonary Resuscitation) and training in catheter care, hospice support, dementia care, and Alzheimer's care preferred Familiarity with long-term care practices and working with individuals with disabilities or developmental disabilities Ability to operate mobility aids such as Hoyer lifts safely and effectively Excellent communication skills for collaborating with healthcare teams and engaging with patients empathetically Commitment to maintaining HIPAA compliance to protect patient confidentiality Join us in delivering exceptional home health services that empower individuals to live comfortably and independently.
