In this vital role, you will provide compassionate, person-centered support that promotes independence, community engagement, and overall well-being. Your enthusiasm and proactive approach will help create a positive environment where individuals can thrive and reach their full potential. This position offers flexible hours and the opportunity to work closely with diverse populations, including those with developmental disabilities. Duties Assist individuals with daily living activities such as grooming, dressing, meal preparation to ensure their comfort and safety. Support participation in community outings, recreational activities, and social events to foster social skills and community integration. Collect and document data related to progress, behavior patterns, and health status to support care planning and reporting requirements. Drive clients safely to appointments, activities, or community events while adhering to all safety protocols. Collaborate with families, case managers, educators, and healthcare professionals to develop and update personalized support plans aligned with each individual's goals. Requirements Strong communication skills and the ability to adapt approaches based on individual needs; familiarity with behavioral management techniques is beneficial. Knowledge of assisting with Activities of Daily Living (ADLs), medication administration, and home & community care practices. Ability to work independently and as part of a team in a dynamic environment; excellent organizational skills for data collection and documentation. Valid driver's license with a clean driving record for transportation duties; willingness to drive clients safely in personal or agency vehicles. Certification in CPR/First Aid preferred; willingness to obtain training related to medication administration or other specialized skills required by the role.
