Job Description

Overview Join our compassionate healthcare team as a Home Health Aide (HHA), where your dedication and caregiving skills will make a meaningful difference in the lives of patients in their homes and community settings. As an HHA, you will provide essential personal care, assist with daily activities, and support patients in maintaining their independence and well-being. This role offers a dynamic environment where your efforts directly enhance quality of life for individuals with diverse needs, including seniors, those with disabilities, or individuals recovering from illness. Duties Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting to promote comfort and dignity Monitor patient health by observing vital signs, reporting changes, and maintaining accurate medical documentation using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems Support medication administration following prescribed protocols while adhering to HIPAA regulations to ensure patient privacy and safety Provide specialized care for patients with dementia, Alzheimer's disease, or other memory impairments through tailored memory care techniques Operate medical equipment such as Hoyer lifts, catheters, and other assistive devices safely and effectively Assist with mobility using tools like Hoyer lifts or transfer belts to prevent falls and promote independence Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe environment for the patient Document all patient interactions accurately in medical records and communicate effectively with healthcare teams regarding patient status Support hospice care patients by providing comfort measures and emotional support during end-of-life stages Engage in ongoing training on medical terminology, CPR certification, and best caregiving practices to enhance service quality Experience Prior experience working in assisted living facilities, nursing homes, or home health environments is highly preferred Demonstrated ability to perform patient observation with attention to detail and compassion Experience working with individuals who have developmental disabilities or require specialized dementia or Alzheimer's care Familiarity with EMR/EHR systems for medical documentation and record keeping Knowledge of infection control procedures, HIPAA compliance, and safety protocols in home health settings Long-term care experience that includes assisting with ADLs, medication management, and vital signs monitoring is advantageous Proven ability to work independently while collaborating effectively within a multidisciplinary healthcare team Join us as a dedicated Home Health Aide where your caring nature fuels positive outcomes.
