This role is essential in providing support and assistance to individuals with varying needs, ensuring their well-being and promoting their independence. Duties Assist individuals with daily living activities, including personal hygiene, meal preparation, and medication management. Implement and monitor care plans tailored to each individual's needs, ensuring adherence to prescribed guidelines. Provide patient monitoring to assess health status and report any changes or concerns promptly. Maintain a clean and safe environment for clients, ensuring that all areas are hygienic and organized. Support individuals in community integration activities, fostering social interaction and engagement. Assist with memory care techniques for clients experiencing cognitive challenges, including dementia care strategies. Uphold confidentiality standards in accordance with HIPAA regulations while documenting care activities accurately. Collaborate with healthcare professionals, families, and other caregivers to coordinate comprehensive support services. Experience Previous experience in home community care, long-term care, or similar settings is preferred. Familiarity with patient monitoring techniques and the ability to implement care plans effectively. Certification in CPR is highly desirable; training may be provided for qualified candidates. A background in nursing or related healthcare fields is a plus but not mandatory. Strong interpersonal skills with the ability to communicate effectively with clients and their families.
