Job Overview Join our dedicated healthcare team as a Home Health Aide, where your compassionate care and attention to detail will make a meaningful difference in the lives of individuals requiring assistance with daily activities and medical needs. In this vital role, you will provide personalized support in patients' homes, ensuring their safety, comfort, and well-being. Your expertise in patient observation, medication administration, and mobility assistance will help promote independence and improve quality of life for those you serve. This position offers an engaging opportunity to work closely with diverse populations, including seniors, individuals with disabilities, and those receiving hospice or dementia care. Responsibilities Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using equipment such as Hoyer lifts or wheelchairs. Monitor and document vital signs like blood pressure, pulse, temperature, and respiration rates accurately using Electronic Health Record (EHR) systems. Support medication management by administering prescribed medications following medical terminology guidelines and HIPAA privacy standards. Observe patient conditions closely to identify changes in health status or behavior, especially in memory care or dementia settings. Provide companionship and emotional support while engaging patients in appropriate activities to promote mental well-being. Maintain a clean and safe environment by assisting with light housekeeping tasks such as laundry, cleaning, and meal preparation to meet nutritional needs. Document all care activities thoroughly in medical records and communicate effectively with healthcare teams regarding patient progress or concerns. Skills Proven experience working with individuals with developmental disabilities, dementia (including Alzheimer's disease), or in hospice care environments. Strong knowledge of medical terminology, vital signs measurement, medication administration, catheter care, and CPR certification. Familiarity with EMR (Electronic Medical Records) and EHR systems for accurate documentation of patient information. Ability to perform patient observation diligently and respond promptly to emergencies or changes in condition. Experience working in assisted living facilities, nursing homes, or home health settings demonstrating long-term care expertise. Compassionate caregiving skills combined with excellent communication abilities to connect effectively with patients of all ages and backgrounds. Physical stamina to assist with mobility tasks such as transfers using Hoyer lifts or other assistive devices while maintaining safety protocols.
