Job Description

Job Summary Join our dedicated healthcare team as a Home Health Aide, where you will play a vital role in providing compassionate, personalized care to individuals in their homes. Your energetic and attentive approach will help clients maintain independence, improve quality of life, and feel supported through tailored assistance. This rewarding position offers the opportunity to work closely with patients of all ages, including seniors, individuals with disabilities, and those recovering from illness or injury. You will deliver essential services that promote health, safety, and comfort in a respectful and dignified manner. Duties Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts when necessary Monitor patient health by observing vital signs, assessing physical and emotional well-being, and reporting changes promptly Administer medications accurately following medical documentation protocols while adhering to HIPAA regulations to ensure patient confidentiality Support patients with medical procedures including catheter care, wound care, and hospice or dementia-related needs such as Alzheimer's or memory care Maintain detailed and accurate medical records using Electronic Health Record (EHR) systems and document patient progress regularly Prepare nutritious meals and assist with feeding while considering dietary restrictions and preferences Perform light housekeeping tasks such as cleaning, laundry, and maintaining a safe environment to promote overall well-being Provide companionship and emotional support to foster positive relationships and enhance mental health for individuals with developmental disabilities or chronic conditions Experience Prior experience in home health care or assisted living environments is highly preferred Demonstrated ability to work with individuals with disabilities, dementia, Alzheimer's disease or in hospice settings Knowledge of medical terminology and procedures related to patient care is essential Experience operating EMR (Electronic Medical Records) or EHR systems for accurate documentation Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as medication administration or hospice care are advantageous Long-term care experience or working within nursing homes adds valuable insight into patient needs Familiarity with medical equipment like Hoyer lifts, vital signs monitors, and mobility aids enhances your effectiveness in delivering safe care Join us in making a meaningful difference every day!
