Job Summary Join our dedicated healthcare team as a Home Health Aide (HHA), where your compassionate care and attention to detail will make a meaningful difference in the lives of patients receiving home and community-based care. In this vital role, you will provide personalized assistance to individuals with diverse needs, promoting independence and enhancing quality of life. Your expertise in patient observation, medication administration, and assisting with activities of daily living (ADLs) will ensure our clients receive safe, respectful, and comprehensive care in the comfort of their homes. This position offers an engaging opportunity to work closely with patients, families, and healthcare teams to deliver exceptional home health services. Duties Provide hands-on assistance with activities of daily living such as bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts when necessary. Monitor and document patient vital signs including blood pressure, pulse, respiration, and temperature using Electronic Health Record (EHR) systems to ensure accurate medical records. Assist with medication administration following prescribed schedules while adhering to HIPAA regulations to maintain patient confidentiality. Observe patient conditions closely, noting changes in health status or behavior related to dementia, Alzheimer's disease, or other cognitive impairments; report findings promptly to supervising nurses or healthcare providers. Support patients with specialized care needs such as catheter care, hospice support, or memory care routines tailored for individuals with developmental disabilities or long-term care requirements. Prepare nutritious meals and assist with meal planning that aligns with dietary restrictions; perform light cleaning and laundry tasks to maintain a safe and hygienic environment. Utilize EMR (Electronic Medical Records) systems efficiently for documentation of medical records and care activities; ensure compliance with all safety protocols including CPR certification when applicable. Skills Strong patient observation skills with the ability to recognize subtle changes in health or behavior. Experience working in assisted living facilities, nursing homes, or home health environments demonstrating adaptability across various care settings. Proficiency in medical terminology and documentation within EMR/EHR systems to accurately record patient information. Knowledge of ADLs assistance techniques including bathing, dressing, grooming, meal preparation, and mobility support. Familiarity with dementia care practices such as memory care strategies and Alzheimer's disease management. Ability to perform vital signs measurement accurately and respond appropriately to emergency situations like hospice or medical crises requiring CPR. Competence in handling specialized equipment such as Hoyer lifts or other mobility aids safely and effectively. Experience working with individuals who have developmental disabilities or disabilities requiring tailored caregiving approaches.
