Direct Support Professional

POSITION GOAL/FUNCTION

Responsible for training individuals with disabilities or other employment barriers in behavioral and social-skills development, vocational skills, and positive work ethics, in accordance with Individual Employment Plans and Individual Service Plans.

ESSENTIAL FUNCTIONS

(ILLUSTRATIVE ONLY) Timely and consistent attendance. Supports agency direction in promoting the community first for services needed. Plan and implement monthly calendars. Use positive techniques and respect when following behavior support strategies. Implement crisis prevention intervention techniques, as needed. Assist individuals to know their rights, meet their goals and provide support as needed. Assist individuals with activity daily living skills, such as, but not limited to, toileting, feeding and mobility. Use necessary assistive devices and equipment to assist individuals' activities, if needed. Complete daily service documentation. Participate in person centered planning process with individuals. Complete all required trainings as assigned. Implement the Individual Service Plan (ISP) as written. Work with individuals to promote socialization and establish relationships with peers by assisting them with desired recreational and social activities within their surrounding community. Maintain confidentiality of information relating to individuals in accordance with HIPAA policies. Treat individuals, families, co-workers and outside parties with respect and dignity. Maintain safety of individuals at all times. May be responsible to drive or provide direct service while on buses. Performs related duties as required. REQUIRED KNOWLEDGE, SKILLS & ABILITIES

(KSAs): Any combination of education and experience.

Knowledge of:

DODD rule, Autism spectrum Disorder, behavior support techniques and strategies, person centered programming, transportation safety.

Skills in:

Interacting with people; communicate verbally and in writing/typing. Basic computer skills.

Ability to:

Read, write and understand English. Lift up to 25 pounds frequently; must have full range of motion in upper body; bending and lifting required. Ability to recognize unusual or threatening conditions related to persons with disabilities. Stand, sit, or walk for lengthy periods of time. Ability to work with others. Ability to observe and document behavior, incidence and occurrences.

QUALIFICATIONS/LICENSE

REQUIRED

High school diploma or equivalency. As required by law or rule, must successfully pass a criminal background check through BCII, Nurse Registry check and DODD Abuse Registry check and others as specified by law or rule. Must successfully complete pre-employment physical exam including drug test. Must complete annual DODD provider trainings. CPI Certification within 90 days of hire. Must pass Motor Vehicle insurance requirements. Must maintain driving record in accordance with Bloom Community Services policy. Must successfully complete 180-day probation period.

EQUIPMENT USED

Computer, assistive technology, maxi lift and other assistive apparatuses, augmented communication devices.

Working conditions may exist that are not such as normally exist in an office environment. These conditions may include exposure to bloodborne pathogens, communicable diseases, potentially infectious material, potentially hazardous materials, and/or aggressive behavior.
