Responsibilities Assist patients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary Monitor and record vital signs accurately, including blood pressure, pulse, temperature, and respiration rates Support medication administration following prescribed protocols while maintaining strict adherence to HIPAA regulations and medical documentation standards Observe and report patient conditions promptly to healthcare teams, noting any changes or concerns related to health or safety Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory impairments through tailored memory care techniques Assist with medical procedures such as catheter care and hospice support as directed by healthcare professionals Perform light housekeeping tasks including cleaning, laundry, meal preparation, and maintaining a safe environment for clients Utilize Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to document patient interactions accurately and efficiently Support clients in community settings by helping them access resources and participate in social activities that promote well-being Skills Proven experience in home health care or long-term care environments with a focus on senior care and assisted living settings Strong knowledge of medical terminology, medication administration protocols, and patient safety standards Excellent patient observation skills to detect changes in condition or behavior promptly Ability to work effectively with individuals with developmental disabilities or cognitive impairments such as dementia or Alzheimer's disease Competency in using medical equipment like Hoyer lifts and performing vital signs assessments Familiarity with EMR/EHR systems for accurate medical documentation and record keeping CPR certification and training in hospice care practices are highly desirable Compassionate caregiver with excellent communication skills who can build trusting relationships with clients and their families Join us to provide exceptional home health services that empower individuals to live healthier, happier lives.
