What You'll Be Doing Conducting psychosocial assessments in patients' homes Developing and implementing care plans Supporting patients and families with emotional and practical needs Educating on advance directives and care planning Coordinating with community resources and services Participating in interdisciplinary team meetings Providing support across hospice (with occasional home health collaboration) Pay & Benefits $45.00
