Portland, OR Job Description We are seeking a Licensed Master Social Worker (LMSW) for a Home Health assignment serving East Portland and surrounding areas. The ideal candidate will have skilled home health experience, strong patient care coordination abilities, and experience working independently in the field.

Key Responsibilities:

Provide social work services to home health patients in community settings Conduct patient assessments and develop care plans Coordinate resources and support services for patients and families Maintain accurate documentation within EMR systems Manage daily travel to patient homes within assigned coverage areas

Requirements:
