About the Position The Home Care Case Aide plays a key support role in ensuring the efficient, organized, and compliant coordination of home care services. This part-time position provides administrative and field support, including preparation for supervisory visits, client check-ins, assistance with assessments and intakes, and care plan documentation, under the guidance of Home Care Supervisors, Regional Managers, and/or designated leads. This role is designed to support the functions of Home Care Supervisors and Branch/Regional Managers under their direct supervision. The Home Care Case Aide does not independently manage caseloads or assume full supervisory responsibilities, but instead contributes to the overall effectiveness, continuity, and quality of client care Essential Duties and Responsibilities Provide direct administrative and field support to Home Care Supervisors and/or Branch/Regional Managers in day-to-day care coordination activities. Assist with scheduling, preparation, and completion of required supervisory home visits, including documentation and follow-up deadlines. Prepare, organize, and maintain documentation and materials needed for supervisory visits and care coordination activities. Conduct client and caregiver check-in and follow-up calls, as directed, ensuring timely communication and service continuity. Collect and document feedback related to service delivery, caregiver performance, and changes in client condition. Accurately document all interactions and promptly escalate concerns, risks, or service issues to the Home Care Supervisor, Regional Manager or lead. Assist with scheduling and coordinating supervisory home visits, including logistical support and calendar management. Attend home visits, as needed/directed, to support documentation, observation, and accurate notetaking. Develop draft documentation, summaries, and reports for review and final approval by the Regional Manager. Support Home Care Supervisors and/or Branch/Regional Managers with new client intakes, assessments, and onboarding processes. Prepare intake packets and ensure all required forms and documentation are completed accurately and in full. Organize and compile intake and assessment documentation for review and finalization. Assist in drafting, updating, and maintaining client care plans in alignment with supervisory direction. Ensure care plans reflect authorized services, client preferences, safety considerations, and care needs. Submit all care plans for final review and approval by the Home Care Supervisors and/or Branch/Regional Managers. Collaborate with management and sales teams to support the creation and distribution of marketing and outreach materials. Support marketing initiatives, including tracking participation, assisting with goal development, and contributing to performance metrics. Benefits and Perks:

$24.00-$26.50 per hour depending on experience
