This role involves supporting individuals with daily activities, medical needs, and emotional well-being in a variety of settings, including private residences, assisted living facilities, and nursing homes. The ideal candidate will possess a strong commitment to patient care, excellent communication skills, and the ability to work independently while maintaining professionalism and empathy.

19013, 19104, 190111, 19018, 19113 , 19083 Any of this zip cod call the office 610-461-1086 ASAP Duties Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support Monitor patient health by observing vital signs and reporting changes to healthcare professionals Administer medications accurately following prescribed instructions and document medical records accordingly Support patients with memory care needs, including Alzheimer's and dementia care strategies Operate EMR/EHR systems for accurate documentation of patient interactions and medical information Assist with catheter care, wound care, and other specialized medical procedures as trained Provide companionship and emotional support to enhance quality of life Prepare nutritious meals tailored to dietary restrictions and assist with meal planning and preparation Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment Use equipment such as Hoyer lifts safely to assist with patient transfers Follow HIPAA guidelines to ensure patient confidentiality at all times Support hospice care patients with compassion and respect for their dignity Experience Prior experience in home health or community care settings preferred Experience working with seniors, individuals with developmental disabilities, or long-term care residents highly desirable Familiarity with assisted living facilities, nursing homes, This role offers an opportunity to make a meaningful difference in patients' lives while developing valuable skills in healthcare.
