Compensation & Benefits:
Pay Rate:
$20.50/hour (fixed)
Mileage Reimbursement:
$0.70 per mile Weekly pay (salary + travel reimbursement)
Duration:
12+ months (Contract-to-Hire)
Job Overview:
We are hiring an Enrollment Case Manager (ECM) to support individuals applying for Home and Community-Based Services (HCBS) programs. This is a field-based role, where you will work directly with applicants in their homes across Lancaster County, helping them complete the enrollment process and access needed services. Requirements (Must Have): Valid driver's license + personal vehicle Willingness to travel within Lancaster County Public transportation cannot be used for this role
Key Responsibilities:
Travel throughout Lancaster County for in-home visits Conduct intake and assessment visits with applicants Guide individuals through the HCBS enrollment process Explain program services, rights, and responsibilities Assist with application completion and data entry (Compass system) Help applicants choose appropriate: Managed Care Organizations (MCO) Providers / Support Coordinators Build relationships with healthcare providers, community partners, and local agencies Track application status and follow up on pending cases Attend team meetings and provide availability updates Occasionally lift/move items up to 25 lbs Perform other duties as assigned Strong communication and people skills Comfortable working independently in the field Background in case management, healthcare, social work, or customer service preferred Basic computer/data entry skills Why Join Us?
