As a Direct Support Professional at LNL, you will partner with individuals with developmental disabilities to maximize their independence, support their health and safety, and facilitate meaningful community participation. You will work in home and community settings, implementing individualized supports and building relationships based on trust, dignity, and choice. You'll be doing more than "care" — you'll become part of someone's life, supporting growth, autonomy, and connection. Key Responsibilities & Tasks:
Below are typical duties; assignments may vary depending on the individual's needs and support plan. Daily Living & Health Support Assist with personal care: bathing, grooming, hygiene, dressing, toileting Support with meal planning, food preparation, feeding, and dietary needs Assist with household tasks: cleaning, laundry, organizing, maintaining a safe home environment Help with mobility, transfers, and positioning where needed Administer or assist with medications, following all protocols and under delegating supervision Monitor and report health concerns, changes, or injuries promptly Behavior, Support Plans & Safety Implement each person's Individual Support Plan (ISP), Behavior Support Plan (BSP), and agreed interventions Use positive behavior support techniques, de-escalation, and trauma-informed approaches Maintain safety protocols (fire, crisis management, emergency response) and support environmental accommodations Document all incidents, shift notes, progress reports, logs, as required by Pennsylvania ODP (6400/6100) Community Integration, Choice & Empowerment Facilitate and support community activities (recreation, volunteer, outings, social) Assist in job coaching, employment support, or educational programming if applicable Support development of skills (budgeting, communication, decision-making, social) Encourage self-advocacy, personal goals, and meaningful choice Accompany individuals to appointments, errands, and community events Communication & Collaboration Work collaboratively with other team members, supervisors, case managers, families, and allied professionals Participate in team meetings, training, and supervision Share observations, feedback, and suggestions to improve supports Qualifications & Requirements Must Have (Required): High school diploma or equivalent (GED acceptable) Valid driver's license and reliable transportation Ability to pass required background checks, state clearances Physical ability to lift up to ~50 lbs and assist with transfers Strong interpersonal skills: patient, compassionate, respectful, and communicative Preferred / Plus:
