This rewarding role offers an opportunity to provide vital assistance in various settings, including assisted living facilities, group homes, and home care environments. Your efforts will promote independence, dignity, and well-being for those you serve, creating a positive impact every day. Duties Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and toileting to ensure comfort and hygiene. Support patients with mobility needs using equipment like Hoyer lifts and assist with transfers safely and effectively. Monitor patient health by taking vital signs, observing changes in condition, and reporting findings accurately to healthcare professionals. Prepare nutritious meals tailored to individual dietary requirements and assist with meal planning and cooking. Maintain a clean and safe environment by performing light housekeeping tasks including cleaning patient areas and laundry. Provide companionship and emotional support to foster social engagement and reduce feelings of isolation. Follow care plans meticulously, including medication administration under supervision, documentation of patient progress, and adherence to HIPAA privacy regulations. Requirements Proven experience in home health, assisted living, nursing home care, or working with individuals with developmental disabilities or dementia care. Certification in CPR and First Aid is required; additional certifications such as HIPAA training are preferred. Strong knowledge of patient observation techniques, medication management, and vital signs monitoring. Ability to perform heavy lifting safely when assisting with mobility or transfers; familiarity with Hoyer lifts is a plus. Excellent communication skills to collaborate effectively with healthcare teams, patients, and families. Experience working with diverse populations including seniors, individuals with autism or disabilities, and those requiring memory or hospice care.
