Job Description

Job Summary Join our dedicated team as a Home Health Aide & Caregiver, where you will play a vital role in enhancing the quality of life for individuals requiring assistance with daily activities and medical needs. This rewarding position offers the opportunity to provide compassionate, personalized care in home and community settings, ensuring safety, comfort, and dignity for each client. As a key member of our caregiving team, you will utilize your skills to support patients with diverse needs, including seniors, individuals with disabilities, and those recovering from illness or injury. Your proactive approach and empathetic nature will make a meaningful difference every day. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools like Hoyer lifts or other assistive devices. Monitor patient health by observing vital signs such as blood pressure, pulse, temperature, and respiration; document findings accurately in electronic health record (EHR) systems while maintaining HIPAA compliance. Administer medications and manage medication schedules following medical documentation protocols under supervision or as trained. Provide specialized care for clients with dementia or Alzheimer's disease through memory care techniques and tailored activities to promote cognitive engagement. Support clients with medical procedures such as catheter care, wound dressing changes, or hospice-related comfort measures. Prepare nutritious meals aligned with dietary restrictions and assist with meal planning and preparation to promote overall well-being. Maintain a clean and safe environment by performing light housekeeping tasks including laundry, cleaning, and organizing personal spaces. Document all patient interactions accurately in EMR (Electronic Medical Records) or EHR systems; ensure confidentiality and compliance with HIPAA regulations. Collaborate effectively with healthcare teams to coordinate care plans and communicate any changes in patient condition promptly. Provide emotional support and companionship to foster a positive environment that respects the dignity of each individual. Experience Prior experience working in assisted living facilities, nursing homes, or home health settings is highly preferred. Demonstrated ability to observe patient conditions closely through patient observation skills. Experience working with individuals with developmental disabilities or dementia enhances your ability to deliver personalized care. Knowledge of medical terminology, vital signs measurement, medication administration, and medical documentation practices is essential. Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as hospice care training or catheter care are advantageous. Familiarity with EMR/EHR systems for accurate record keeping; experience with home & community care environments is a plus. Long-term care experience and working with patients who have complex needs will strengthen your application.
