Job Overview Join our compassionate team as a Home Health Aide, where your dedication and skills will make a meaningful difference in the lives of individuals needing personalized care. In this vital role, you will provide essential support to clients in their homes, assisting with daily activities, health monitoring, and emotional well-being. Your energy and positive attitude will help promote independence and improve quality of life for those you serve. This position offers an opportunity to work closely with diverse populations, including seniors, individuals with disabilities, and those requiring specialized care such as Alzheimer's or dementia support. Duties Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment like Hoyer lifts. Monitor and record vital signs such as blood pressure, pulse, temperature, and respiration to ensure health stability. Support medication administration following proper protocols and documentation in Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems while maintaining HIPAA compliance. Provide companionship and emotional support to promote mental well-being, especially for clients with memory care needs or living with dementia or Alzheimer's disease. Perform light housekeeping tasks including cleaning, laundry, meal preparation, and ensuring a safe home environment. Document medical records accurately and communicate effectively with healthcare teams regarding patient progress or concerns. Assist with medical procedures such as catheter care or hospice-related tasks under supervision when necessary. Requirements Proven experience in home health care, assisted living facilities, nursing homes, or long-term care environments. Strong knowledge of medical terminology and caregiving best practices for dementia, Alzheimer's care, hospice support, and working with individuals with developmental disabilities. Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as medication administration or catheter care are preferred. Ability to operate EMR/EHR systems efficiently for documentation purposes while adhering to HIPAA regulations. Experience working with patients who have complex needs including memory care or mobility challenges; familiarity with Hoyer lifts and other assistive devices is advantageous. Excellent patient observation skills to monitor health status and identify potential issues promptly. Compassionate attitude combined with strong communication skills to foster trust and rapport with clients and their families.
