Job Overview Join our dedicated healthcare team as a Home Health Aide, where your compassionate care and attention to detail will make a meaningful difference in the lives of individuals needing support at home. In this vital role, you will provide personalized assistance to clients, helping them maintain independence, comfort, and dignity. This position offers an engaging opportunity to work closely with diverse populations, including seniors, individuals with disabilities, and those requiring specialized memory or hospice care. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor patient health by observing vital signs such as blood pressure, pulse, temperature, and respiration; report changes to healthcare professionals promptly Support medication administration following prescribed guidelines while maintaining accurate medical documentation in Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems Provide specialized care for clients with dementia, Alzheimer's disease, or other memory impairments through tailored memory care techniques Operate medical equipment such as Hoyer lifts and catheters safely and effectively; assist with transfers and mobility using appropriate assistive devices Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe living environment in compliance with HIPAA regulations Document all patient interactions thoroughly in medical records while respecting patient confidentiality and privacy standards Skills Strong patient observation skills to identify changes in condition or behavior Experience working in assisted living facilities, nursing homes, or home health environments Familiarity with EMR/EHR systems for accurate medical documentation Knowledge of medical terminology relevant to home health care settings Ability to assist with complex care needs such as hospice support, catheter care, and medication administration Proficiency in vital signs measurement and basic nursing tasks within long-term or home care settings Compassionate caregiving attitude with experience working with individuals with developmental disabilities or dementia-related conditions CPR certification and understanding of safety protocols for patient handling and emergency response Ability to perform physical tasks involving lifting (Hoyer lift), cleaning, cooking, and assisting with laundry while maintaining a safe environment Excellent communication skills to collaborate effectively with clients' families and healthcare teams Join us in delivering exceptional home health care that enhances quality of life through dedicated service.
