Role:

We are seeking a dedicated and caring individual to provide non-medical assistance to our patients with daily living activities, including meal prep , medication reminders , bathing , and medication reminders . As a Home Health Aide, you will work closely with our care team to develop personalized care plans that cater to each patient's unique needs and preferences.

Responsibilities:

Provide non-medical assistance with daily living activities, including meal prep , medication reminders , bathing , and medication reminders . Assist patients with grooming, dressing, and personal hygiene. Help patients with mobility and transfers, as needed. Monitor vital signs and report any changes to the care team. Maintain accurate and detailed records of patient care and services provided. Collaborate with the care team to develop and implement individualized care plans. Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions.
