Home Health Aide

Duties Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility Monitor and observe patient health status, including vital signs and overall well-being Support medication administration following proper protocols and documentation standards Help with light meal preparation, feeding, light housekeeping, laundry, and cleaning to maintain a safe and comfortable environment Utilize Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to accurately document patient care activities and medical records Provide specialized care for individuals with dementia, Alzheimer's disease, or other memory-related conditions Assist with transfers using Hoyer lifts or other mobility aids while ensuring patient safety and comfort Requirements Must Care For People Like Your Own Family Proven experience in home health care, assisted living, nursing homes, or long-term care environment Ability to work independently while following detailed care plans and protocols Strong patient observation skills to detect changes in condition or behavior Experience working with individuals with disabilities or dementia is highly valued Familiarity with EMR/EHR systems for accurate recordkeeping is a plus Compassionate attitude combined with excellent communication skills to build trust with patients and their families ID/D Support in home and the communityJoin us in providing exceptional home health services that prioritize dignity, independence, and well-being.
