Job Description

Overview Join our dedicated healthcare team as a Home Health Aide & Caregiver, where your compassion and skills will make a meaningful difference in the lives of individuals requiring personalized care. In this vital role, you will provide essential support to patients in their homes and community settings, promoting independence, comfort, and well-being. Your energetic approach and attention to detail will help create a safe, nurturing environment for those in need of assistance with daily activities and medical care. Duties Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools such as Hoyer lifts when necessary Monitor patient health by observing vital signs, managing medication administration, and documenting medical records accurately within Electronic Health Record (EHR) systems Support patients with memory care needs, including Alzheimer's and dementia care, ensuring safety and emotional comfort Perform medical procedures such as catheter care, wound care, and hospice support while adhering to HIPAA regulations for patient privacy Assist with meal preparation, feeding, light housekeeping, laundry, and cleaning to maintain a safe and hygienic environment Use medical terminology appropriately when documenting patient information and communicating with healthcare teams Work with individuals with developmental disabilities or disabilities to promote independence and quality of life in home or community settings Experience Prior experience in home health care or assisted living environments preferred Knowledge of long-term care practices including hospice, dementia care, and memory support techniques Familiarity with EMR/EHR systems for accurate documentation of patient interactions and medical records Hands-on experience working with seniors or individuals with disabilities in residential or community-based settings CPR certification required; additional certifications such as medication administration, catheter care, or hospice care are a plus Experience operating medical equipment like Hoyer lifts or vital sign monitors is highly desirable Strong understanding of medical terminology and patient safety protocols to ensure high-quality caregiving Join us in providing compassionate, professional home health care that empowers individuals to live their best lives. Pay:
$18.00 - $22.00 per hour
